Who Qualifies for Hospice?

  • Those diagnosed with a life-limiting condition with a poor prognosis should the disease follow it's natural course  

​

  • Those with illnesses in which curative measures are no longer effective, appropriate, or desired

​

  • Families and caregivers of these loved ones

It is not necessary to be homebound to receive hospice care.  

Persons with any advanced illness are eligible.

Some common diagnosis include:

​

ALS . Alzheimer's . Cancer . Congestive Heart Failure .

End-Stage Liver Disease . End-Stage Renal Disease . Multiple Sclerosis . Stroke .

Parkinson's Disease . Pulmonary Disease (COPD)

​

 

In addition, those experiencing serious multi-symptom deterioration, frequent emergency room visits and/or hospitalizations, or discontinuing aggressive treatment may also be eligible for hospice care.
